Patients wishing to register with the practice are required to complete a registration form and bring their medical cards (if available), a form of photo ID and a recent proof of address (within the last 3 months). You will also be requested to complete other NHS paperwork at the reception desk. You will be given information about the practice (leaflet) and booked to attend a health check. The practice needs your past and present medical history, as many weeks can pass before your paper medical records reach us. All new patients will be registered with a GP and issued with online login details.
